INSTRUCTIONS FOR USING THE ELECTRONIC DEGREE VERIFICATION FORM

A copy of the Degree Verification Form should accompany the Payroll 100 form for all new faculty appointments where no other evidence of degree completion is available (copy of diplioma, for example). This form may also be used with the 100 form for any other position where a college degree is required.

The verification may be obtained by calling the Registrar's Office at the institution issuing the degree and asking for verification. The NDSU caller then completes the verification form and signs it.

The form may be completed on-line in Adobe or saved as a Wordperfect file.
