To submit a case / evidence to the NDSU Forensic DNA Facility please follow these steps:

• Contact the Forensic DNA Facility via telephone or email to create a new case file.
• Fill out and print the Case Submission form and include it with evidence that is to be submitted. Please remember that a fully completed Case Submission form is required with all evidence submitted.
• Ship materials / evidence to the NDSU Forensic DNA Facility.
